This document explores the NYSACRA Learning Institute’s transformative approach to individualized supports for people with developmental disabilities, held in New York’s Southern Tier from June to November 2015. ​ Supported by the NY State Office for People With Developmental Disabilities, the institute focused on empowering change teams from participating agencies to create person-centered, self-directed services. Through workshops, learning journeys, webinars, and mutual support networks, participants tackled challenges like inflexible systems, fear-driven practices, and outdated assumptions about people’s capabilities.

The institute emphasized the importance of listening deeply, questioning limiting beliefs, and co-creating solutions with individuals. ​ Using frameworks like Theory U and Blue Space, teams learned to prototype innovative supports, such as customized housing, assistive technology, and meaningful employment opportunities. Design principles highlighted the need to honor individual choice, minimize social distance, and reinvest resources away from congregate services toward personalized supports. ​

The document also addresses systemic barriers, such as rigid funding models and risk-averse cultures, while offering actionable strategies to cultivate change. ​ It encourages readers to embrace collaboration, take reasonable risks, and focus on building relationships that empower individuals to lead fulfilling lives in their communities. ​
